Assist in applying for Child Care Home-base Licensing
To participate in the Child and Adult Care Food program (CACFP) an applicant has to be a licensed Child Care Home Base provider. If not, an applicant can apply for a License from the State of Texas Health and Human Services where Texas Kids Nutrition Corporation (TKNC) can assist the applicant through all necessary steps to fulfill state application requirements.
Documentations needed for a licensed Child Care Home Base in Texas:
o Complete the pre-Application courses (Orientation and 30 hours Minimum Basics).
o Complete the Application for a license to operate a Child Day Care Home Base Facility.
o Complete the Fee Schedule form and your Fee payments to the Accounting Department listed on the form.
o Complete the Controlling Person form.
o Complete the Notarized Affidavit for Applicant for employment.
o Complete the Child Care Licensing Governing Body/ Director designation.
o Complete the Personal History Statement.
o Complete the Plan of Operation.
o Complete the Verification of Liability Insurance.
o Submit the proof of High School Diploma or GED for applicant. If in foreign language, must be translated to English and a copy of Texas Driver’s License.

o List of Registered children at CCHB facility
